Summer 2008 Brochure Summer 2008 Registration Form Camper Health History Forms: The Association for Camp Nurses (ACN) and the American Academy of Pediatrics (AAP) recently joined together to develop two new medical forms for camps to use with their camper population. Form 1 is a four-page camper health history; Form 2 is a one-page medical recommendation form to be completed by physicians. We are transitioning to these two new forms, which used together are equivalent to the old “Green Form.” Either the new or old forms are appropriate for this summer.
